Yes. Your company purchases a perpetual licence to use the app within the licensed Microsoft 365 tenant. The purchased version can be used long-term with all included features, without a time limit or subscription. This right continues after the included year of support ends. Required Microsoft 365 / SharePoint access and its costs remain separate.
You can try Jelly Forms free for one month. Afterwards, a perpetual licence for one Microsoft 365 tenant costs €4,999 as a one-time purchase, plus applicable VAT. One year of product support is included from the date of purchase.

Jelly Forms customises the New, Edit and View forms of modern generic SharePoint lists. Document libraries are not part of its intended scope.
The designer requires the Manage Lists permission on the target list. Publishing also requires permission to write the form configuration. Normal form use follows each user’s SharePoint permissions.
No. The export captures the values currently displayed, including unsaved input. Save the form separately to update the SharePoint list item.
The product controls are available in English and German. Your custom field labels, choices and authored content remain under your control and are not translated automatically.
